What you attempt depends on your server. If you are very ambitious and have a server that will accept cgi scripts, see the scripts available from the site listed in the "Resources" link and try using one on your site.

If your server that does not accept cgi scripts or you do not want to attempt to adapt it for your server, go to one of the form hosting services such as www.response-o-matic or a service listed at Yahoo or other search engine. Prepare a form and use this service so that you have a form on one of your site pages.

By doing at least one of these preformatted scripts you may get some idea of how they work.
